Over the past 3 decades, there are few areas in the field of medicine where our understanding of pathophysiology has increased more dramatically than in the specialty of allergy and clinical immunology. Our knowledge of the molecular biology and genetics of immune and inflammatory responses is expanding exponentially. As the authors of Allergy: Principles and Practice point out, this new knowledge explosion at the molecular level, as exciting as it is, may be just beginning. The possible outcome of this evolution is much more targeted, precise therapy of allergic and immunologic disease. Writing a textbook in such a rapidly evolving specialty is a daunting task.
Few allergists do not have at least 1 edition of Allergy: Principles and Practice, which is rightly considered the authoritative text in the specialty of allergy and immunology. The authors of the text have constantly strived to update this definitive source to ensure timely, evidence-based information for the clinician and researcher alike. Now in its 6th edition, the most recent version of the text, frequently referred to as “Middleton, Reed, and Ellis,” is once again an exceptionally useful addition to the library of anyone caring for patients with allergic, respiratory, or immunologic disease. Dr. Elliott Middleton, Jr., the founding author of the textbook that still bears his name, passed away in 1999 after a long and distinguished career as both a researcher and a clinician. His esteemed collaborators on the original 1978 version of this textbook included Dr. Charles Reed, past Chairman of the Department of Allergic Diseases at the Mayo Clinic (and an influential mentor of mine), and Dr. Elliot Ellis. This is the first edition of this textbook to be published without the involvement of its 3 founding editors; however, the legacy of these 3 gentlemen will probably continue for many years to come. The current editorial board of Allergy: Principles and Practice includes some of the most prominent and respected clinician/researchers in the specialty of allergy, including Drs. N. Franklin Adkinson, Jr., Bruce S. Bochner, William W. Busse, Stephen T. Holgate, F. Estelle R. Simons, and John W. Yunginger.

The editors state in the preface that the textbook's “original editorial vision has not changed: to select editors who were experienced practitioners and investigators in the field, and whose knowledge in aggregate would be sufficient to pursue and then critically evaluate invited contributions from authorities in all aspects of the discipline, from molecular medicine to genetics, epidemiology, and therapeutics.” They have held true to their mission, with a group of 161 international contributors who have provided often-exhaustively referenced chapters in their respective areas of expertise.
As in the past, there are extensively referenced, state-of-the-art reviews of all aspects of allergic diseases, including asthma, rhinitis, sinusitis, food and drug allergy, and anaphylaxis, are provided. In this most recent edition, there are 10 new chapters addressing the latest developments in allergy and immunodeficiency disorders, with well-referenced perspectives on the immunopathology of airway inflammation, outdoor allergen aerobiology, signal transduction events, antigen processing pathways, apoptosis of inflammatory cells, animal models of asthma, molecular biology of allergens, immunomodulator therapy, nasal provocation testing, and the biology of airway smooth-muscle cells.

The authors have achieved their stated purpose – namely, to provide a tool to facilitate communication between clinicians and researchers. The ultimate beneficiary of this process is the patient. Through improved communication between these groups, more effective, targeted therapy for patients with allergic and immunologic disease can be developed. For this reason, this text continues to be a noteworthy and important contribution to our medical literature.
